Health Information is the lifeblood of the health care delivery system. It is the practice of acquiring, analyzing and protecting digital and traditional medical information vital to providing quality patient care. The patient's medical record contains the medical information that describes all aspects of your patient care at Washington County Hospital & Nursing Home. The Health Information Services Department works to ensure a patient's health information and records are complete, accurate, and protected of the clinical information needed to provide healthcare-related decisions.

Release of Medical Information

In order to receive copies of your medical information, a release form must be completed. A nominal fee will be charged for medical records, excluding for the purpose of continuity of care.
